A key component of improving population health outcomes is effectively targeting educational resources and population health strategies.

Silver Fern’s population health data helps you quickly identify the most common health barriers affecting a targeted population. Like, for example, a group of commercial drivers with prediabetes. Commercial drivers face challenging work schedules and are at high risk of developing type 2 diabetes, which can impact their livelihood and quality of life.

WellSpark Health distributed Silver Fern’s Meal Plan Module to 97 commercial drivers as part of a customized, behaviorally enriched Diabetes Prevention Program. In addition to hyper-detailed, patient-specific assessment results, Silver Fern delivered population-level data to program leaders that were used to tailor training content and implement program-engagement strategies.

Here were the top three meal plan behaviors, meal plan barriers, and meal plan goals among the commercial driver population:

MOST COMMON BEHAVIORS MOST COMMON BARRIERS MOST COMMON GOALS
42%Didn’t limit the number of carbohydrates at each meal41%Had a sweet tooth37%Wanted to eat healthier

24% Ate out at restaurants or bought fast food28% Enjoyed snacking between meals21%Wanted to eat more fruits and vegetables

One of the strategies that WellSpark used with this population was to teach drivers how to analyze restaurant menus, including the key knowledge and skills to identify healthy options and to ask questions when necessary to determine ingredients and nutrition information.
