On November 2nd, Silver Fern released the most insightful and powerful version of its Behavior Diagnostic Platform ever. Version 2.1 of Silver Fern’s software-as-a-service product includes the highly anticipated integration of a new feature, “Suggested Actions”. Silver Fern has developed thousands of Suggested Actions that are evidence-based and have been used successfully by clinicians with patients to address their individual needs and challenges. These actions are tailored to provide guidance and ideas for the specific experiences, behaviors, goals, or barriers illuminated about each patient by Silver Fern assessments. Suggested Actions include educational items important to patient success, strategies for working effectively with the patient, and clinical referrals that may be appropriate based on the patient’s responses. Users can track and report on which Suggested Actions have been completed, increasing the efficiency of documentation.

The Behavior Diagnostic Platform, originally released in the commercial market in 2018, leverages 30 years of clinical research and integrates the American Diabetes Association’s 2016 landmark guidelines for prediabetes and diabetes, which emphasize the importance of addressing behavior psychosocial issues. The Behavior Diagnostic Platform features clinically proven techniques for assessing self-management behaviors, psychosocial factors, and social determinants of health, and provides chronic care management teams with previously inaccessible insights to improve treatment plans and achieve better health outcomes for people with chronic diseases.
Also launched as part of the 2.1 upgrade, Healthy Lifestyle and Disease Prevention joins the company’s growing list of disease programs, which includes Prediabetes, Type 2 Diabetes, and Heart Failure. The Healthy Lifestyle and Disease Prevention program applies Silver Fern’s evidence-based techniques for identifying patients’ individual barriers, goals, and preferences for treatment. The new program applies to a broader population of people and is appropriate for use with patients in emerging risk pools.
