Silver Fern on the Road and Sharing Our Insights!

Silver Fern’s team couldn’t wait to attend AHIP 2022 in person this year!
Over the two-and-a-half-day conference, speakers presented on topics such as health equity, virtual care, care at home, personalized medicine, and what all of this means in a post-COVID world.

Here’s a quick summary of some of the key themes raised at AHIP and how Silver Fern can help you tackle them head-on.

The importance of identifying and addressing health disparities.

All Silver Fern assessment modules unlock key patient self-management and psychosocial issues for chronic care teams and meet Medicaid readability guidelines. Our evidence-based SDoH+ survey, for example, has been tested in FQHCs and diverse populations, providing valuable information that can help caregivers identify health disparities, and develop practical treatment plans to address them.

Telemedicine/ Virtual Care is a must moving forward.

As they build innovative virtual care models, clinical teams can build more positive personal interactions with their patients by integrating Silver Fern’s unique patient behavior and psychosocial assessment modules into their team workflows. Targeted modules can be sent out efficiently and at scale to patients via text, email, phone, or video chat and they take just 1-5 minutes. The modules’ accessibility and ease of use by patients help build patient engagement and also provide a more personalized care model.

In a recent pilot study of type 2 diabetes patients, 61% of participants completed assessments sent via text survey. Of those patients completing the assessments, 64% had previously disengaged from treatment for 30+ days. Patients want to receive care at home.

At Silver Fern, we know that the physiological data gathered through remote home monitoring only tells half the patient’s story. Because patients are more than their vitals, our questions prioritize the key self-care behaviors and social aspects of patients living with chronic conditions that complement traditional physiological data. The insights gained from asking the right questions using Silver Fern’s assessment modules open a door to enriched communication and patient engagement in home care programs. Patients are more engaged, adhere to medication regimens better, and are more confident in their ability to manage their health.

Personalized medicine includes tailored meals, wellness, chronic disease prevention, and chronic disease support.

It can be difficult to identify a patient’s more complex behavior and psychosocial needs solely from brief, clinic-based visits, and biomedical data. Silver Fern’s targeted programs help introduce the patient to their care manager at a deeper, more personal level, recognizing individual self-management needs and creating a bridge to lasting lifestyle change.

A nationally representative survey, commissioned by Silver Fern Healthcare found that 97% of Americans are willing to spend time prior to doctor visits answering online surveys about their personal daily health habits if that information allows their provider to help them reach their health goals (e.g., lose weight, eat healthy, prevent or manage chronic disease).

Unlocking patients’ care needs can be as easy as asking the right questions.

Moving Forward with Silver Fern
While addressing patients’ biggest obstacles like healthcare disparities, racism, loneliness, or low motivation to engage in treatment, be sure your solution is also unlocking the patient’s daily self-management barriers and psychosocial issues that are key to delivering effective, whole-person care.
