Silver Fern Wraps Up 2020 Adding Hypertension Program to Its Proven Platform

On December 21, 2020, Silver Fern Healthcare released Hypertension, the newest chronic disease program featured on its proven Behavior Diagnostic Platform. This Platform provides chronic care management teams with previously inaccessible insights to improve treatment plans and achieve better health outcomes for people with chronic diseases.

The Hypertension Program applies Silver Fern’s evidence-based techniques for unlocking patients’ individual barriers, goals, and preferences for treatment, and features assessments and clinical decision support tools tailored specifically to patients with hypertension.

Hypertension is the third program released by Silver Fern in the second half of 2020. Silver Fern’s slate of existing disease and pre-disease programs includes: Type 2 Diabetes; Heart Failure; Prediabetes; and Healthy Lifestyle & Disease Prevention.

The Behavior Diagnostic Platform, originally released in the commercial market in 2018, leverages 30 years of clinical research and integrates the American Diabetes Association’s 2016 landmark guidelines for prediabetes and diabetes, which emphasize the importance of addressing behavior psychosocial issues. The 2.2 Behavior Diagnostic Platform release includes additional feature enhancements and content improvements across the Platform.
